As a child, suicide was an unknown option to him. As an adult, unable to cope with the "shame" of his mother's abuse, it's his way of punishing himself. He has yet to realize it was never his fault, his doing or his guilt to bear. That all falls squarely on his mother's shoulders.

It was probably his shaving blade. He was considered nonviolent and they seem to want the patients live as normal a life as possible. In the days before electric shavers, that meant one with a blade, either a two sided that was removable from the handle or a folding straight blade.
