He even left the village, so until chapter 400 he doesn't have any idea of becoming hokage. He only supposedly became the strongest in the world, but I never read anything about him wanting to be a hokage. I think the original author gave the book that title for lack of imagination.

Replied to Grenhaer I'm sorry, but the Chinese author as well as translateotaku don't specify how old he was when he came to Naruto's world. But according to my guess, the novel refers to two situations: The first is that the protagonist transmigrated into his current body when he was discovered by a ninja from Konoha village.
The second is that he was born naturally in the small ninja village.
But I think the second option is more plausible.
He had come to this world for years.
