the letter is cup size while the number is the band size so if using we'll say inches her band is for someone with a "ribcage" (by measuring under the boobs) of 78 inches around, the cup size is then determined by the difference in inches when measuring the "ribcage" and boobs (by measuring over the nipples) so her cup size is D.
From what I remember of the earth's measurements, my mother's breast would be the size 78D.
