I don't think he forgot the fish is his main body/transformation the other 2 is what he can transform into. If/when he can transform into a human it will add to this list. If he was a human that can transform into these animals then it would have been 3 different names there

it was good up until the MC created his own world and the main story got ignored. the name is sign in then it just got ignored for 2/3 of the story. seems like author wrote 2 different storys and combined them for one
