[SPOILERS PROBABLY] After Triain, there wasn't much use of Oppai's power, look at CxC, DxD, AxA, and fairy wyverns, in fact, after unlocking Triain, Issei began to tone down his perversion a lot until Azazel had to guide him back to the "right" path. The only uses of Oppai's power in the later volumes of the novel are when he used Rias's boobs to gain stamina (which was his predecessors' idea) and the announced engine that will probably transform AxA into ExE.

It was never stated in the novel that devils mutate after leaving their master, but it is important to remember that not all devils look human. The example in the picture is on the back.

I checked the wiki again and the description of how the prototype works is "When the Omnitrix in question was activated, it opened up a holographic display of all aliens unlocked. The selected DNA is then merged with the wearer's DNA, transforming the user into the selected alien." but we don't have an example of what would happen if it was disconnected from the Codon Stream because it never happened. However, you probably have the right that traveling to other dimensions would not necessarily mean that the device would not work, "The connection between the Omnitrix and Ultimatrix and the Codon Stream seemed to be remarkably strong, able to traverse dimensions; Ben was able to transform while in the Null Void, outside of the universe in the Forge of Creation, in a parallel universe, and even while trapped in a video game." but it does not change the fact that Azmuth could use the second receiver.
